Free 'Move Together' programme for over 50s being offered by Bucks GPS in 2026

Select GPs in Bucks are offering a free programme to help residents with long-term health conditions to be more active.

Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), Move Together Bucks is a three-year initiative which with personalised guidance and professional support aims to help adults over 50 living with long-term health conditions to move more, feel better and maintain their independence.

The initiative, available to those in the surgeries listed below or those in Opportunity Bucks wards, means those eligible could receive up to 12 months of support which includes: a personalised 12-week activity plan, assistance accessing further local and affordable activity sessions as well as encouragement to boost motivation and confidence.

In a testimony provided on Active in the Community's website, a resident said "I have gone from doing one class a week to 4 classes, plus being more active everyday. I am also going out more, which is really good for my mental health as I live alone, so I have more social contact with people. The best bit about the programme was it gave me the motivation to look at what else is out there. I still occasionally get some pain but since moving more and exercising it's not as severe"

To be eligible, residents must be: over 50, currently doing less than 30 minutes of activity daily, motivated to be more active and living with a long-term health condition (see list below).

However, the initiative notes that those who meet the following criteria can not be considered eligible: 

  • Have had an unplanned or unexpected admission to hospital because of a health condition in the last 4 weeks. If this is the case, seek advice from a healthcare professional before joining Move Together.
  • Have acute coronary syndrome (a heart attack within the last 6 weeks or unstable angina)
  • Have severe or poorly controlled asthma
  • Have severe COPD
  • Are currently accessing cardiac or cancer rehab or pulmonary services
  • Are currently undergoing wider tests regarding the long term health condition(s)
  • Have systolic or diastolic blood pressure levels
  • Have a resting heart rate over 100 bpm
